Supplemental Questionnaire Pursuant to California Public Resources Code Section 5164

Applications for employment or to serve as a volunteer with the City of Yucaipa for a position covered by Public Resources Code Section 5164 (positions involving supervisory or disciplinary authority over any minor) will require you to complete specific supplemental questions through the online application. These supplemental questions will be required for the following positions:  Community Services, Maintenance, Other (if required in the job announcement).
